Want to opt-out or waive benefits insurance? Cash Incentive - If you are covered by other group insurance you may elect to opt-out and receive a cash incentive. See the Insurance Benefits Opt-Out Form. Deadline September 10th. If you don't have other group insurance, but want to waive medical, dental and/or vision insurance, complete the Insurance Benefits Waiver Form.
