Cash Incentive - If you are covered by other group insurance you may elect to opt-out and receive a cash incentive. See the Insurance Benefits Opt-Out Form.

If you don't have other group insurance, but want to waive medical, dental and/or vision insurance, complete the Insurance Benefits Waiver Form.

HRA VEBA offers an account-based plan you can use to reimburse out-of-pocket medical, dental and vision costs. Your account is funded with contributions from your employer.

Available to full-time employees only.
